Hello ladies I need help on filling out my marriage license paperwork as we speak, but it tells me to put my birth name and last name as I did fill that out.. although on the bottom it asks me to put new middle or new last name is that were I put my fiancé‘s last name??? Are you changing your middle and/or last name when you get married? Then you enter your new middle and/or last name where it asks for your new name(s.) If you’re taking your FH’s last name, then yes, that would be your new last name.

You have the option of using your maiden name as your "new" middle name on your marriage certificate.
Example: Susan Jane Smith is marrying John Jones. Her new name can be:
Susan Smith Jones; Susan Jane Jones; Susan Jane Smith-Jones; Susan Jane Smith Jones; or pretty much any other combination you like.
