I had my straps taken up for $50 and a bustle added for $75, total of $125.
I believe adding cups isn't too expensive, but a hem will definitely depend on how many layers your dress has.

I added a bustle, cups, had it taken in around my waist and thighs, hemmed, and shortened the zipper. I paid slightly under $400.

I spent around $700. I added off the shoulder sleeves, a 5 point bustle, had the waist taken in, hem shortened, and cups added in.

$900 for hem, bustle, and taking in the waist and chest area. Total of 4 fittings/sessions.

$500 for hem (4 layers), cups added, and having it taken in at the waist (there's boning and lace so it's kind of complicated).
It probably depends on where you live. I'm near Boston.
