I want a unique title for mine and my fiancée’s best friend (the same person), the catch is she acts as a mediator for us, and honestly is a big reason we are still together. Any ideas?

What role in the ceremony will this person perform, if anything? Officiant, usher, bridesmaid or grooms woman? Decide on that because the role of relationship counselor is not appropriate to advertise to guests in a program. If she is not in the ceremony performing a duty, then she is a guest with no title.

Are you wanting her to serve as a joint best man/maid of honor? You could call her “honor attendant.” But I agree with the previous comment to name based on the wedding role she’s fulfilling.
