rsvp wording

How is everyone wording their RSVP card? Are you putting number of guests attending? I don't really want to give people an option to bring anyone they please...

Any exaples or sites to use as reference is helpful

    I don't have a picture of my invite but here's what i'm getting.

    However I'm not including my email info or anything like that. It's going to look like this:

    Monogram

    ___ graciously accepts

    ___ regretfully declines

    Please indicate number of each.

    ___ roqufort filet

    ___ salmon

    it says this:

    And as you can see, I had additional cards made for larger families, without the design on the bottom, with two extra lines added. I filled in everything except the "accepts" "declines" and "choices" for people, so it was pretty obvious who was invited (even though I had an inner envelope)...however that clearly did not stop his aunt from adding thier dogs, which was actually funny, and his other aunt from adding a sticky note with two more guests even he has never met(which was seriously not funny!!!!)

    Yay! I got a better picture...however apparently FH's parents didnt really catch on to the accepts and declines portion of the rsvp LOL

    What I found odd, was the man who made us choose prime rib apparently did not pick it himself.
