rsvp format. Is this good?

Here is my basic format for the RSVP. Is it okay? Anyone have experience in which the M is confusing to guests? Since I will have the number of seats pre-filled instead of the "2 of 2 happily accepts" format I guess the goal is to have people fill out their names so I want it to be as straightforward as possible. Should I put Name instead?

RSVP Kindly respond by the twentieth of September

We have reserved __ seats in your honor

____ happily accepts____ regretfully declines

For some reason the M____ really confuses people. -__- we put in small print under the line Please provide the full name of those attending.

Yeah I don't know why it confuses people. I think until they get married they don't understand what it means! Haha

We put "name" because we had SO many young single men that would be confused by "M" haha!

That's what the name line is for. They shouldn't write all the names if they all can't come. Or like if mom and dad is coming and their kids aren't.

This is our RSVP Card. I just mailed out invites today but those who I have already hand delivered their invite to were just fine understanding what the M meant. Whichever way you chose I think will be great!

I think the M is confusing and I don't personally like being addressed as "Mrs. XXX" so I always cross it our and write our first and last names.
I just put a line with: Name(s)_______________ instead of the M.

I'm not sure if this applies to your guest list, but if they are doctors or pastors in your group it shouldn't start with M either. Our firm has alot of Dr. & Pastor clients and it is extremely important to address all or their mail correctly. Just putting name could solve this for everyone.

We put "name(s)" instead of the M. Even with the change, some our guests still returned the RSVP without their name on it smh.
