Online RSVP: One form per person?

Hi everyone - We opted for online RSVPs. Our wedding invite includes an insert card that says 'for more info and to rsvp visit www...' and then says 'we reserved you X seats'. For the online form, I am using squarespace. They have a slot for "name" and whether you are attending or not, etc.

My only confusion is whether I should have the form include only one name slot, (meaning each individual fills out an RSVP form), whether to include two slots (most people are coming in couples or were given a plus one), or to include 5 slots for names because a few groups were given an invitation to "X Family" and they are a couple with teenage kids.

My thought is that people might just RSVP themselves as a couple if I do two slots and that the rest of the family might not fill in their RSVPs, so I won't know how many people from these families will be attending. On the other hand, the form is longer with five empty "name" slots, and I don't want anyone to find it confusing.

Thoughts?

    (We're using Wix and a custom db/form for RSVP)

    Personally, one person at a time. Unless you have a huge amount of large households invited, it's the easiest way to do it. Having more than one person per form may also confuse those who have been given X seats reserved - as they may try to add additional people since the form has the space to. (Not sure how your form is set up if it would kick back an error if someone tried this or not)

    I was afraid of people adding plus ones as well, but that is why on our cards we specify how many seats we have reserved for them. I feel like it would be pretty odd for them to add people when their card specifies that, but maybe they would! Our form does not have a way to prevent it.

    On the other hand, I am afraid that if I leave only one slot for "name", that the people who have been given plus ones or who are part of a couple will just RSVP under one person's name and not bother to do another form for their plus one, etc.

    It is very tricky to predict human behavior in this situation! haha

    I don't have any advice. This is good to know since we also have our wedding website on square space and want to offer online RSVP.
