Name etiquette/rule

So I never thought about this before it came up on a medical form. They ask you to choose Miss, Ms., or Mrs. I am married (so a Mrs.), but I did not take my husband‘s last name. So if I choose the Mrs. option, I would be listed as Mrs. <father’s last name> which doesn’t seem right. What is the etiquette/rule surrounding this?? For those of you who kept your maiden name, what do you do in these situations? Do you just choose Ms.?

    I don’t know if there are specific etiquette rules surrounding this, but my guess would be to go with Ms. since it doesn’t signify married state. Because, as your said, Mrs. Last name would really be your mom.

    I'm in a similar situation. I am married, but did not take my husband's last name. From what I found online, either Mrs or Ms would be acceptable for someone who kept their name after marriage. I usually choose Ms.

    This might be not be much of an issue since this is a medical form. Would you even get snail mail addressed to a Mrs. Cece or Ms. Cece?

    In my circles, we address all those identifying as women as "Ms." Unless they introduce themselves first as "Mrs." Personally, I don't think it's anyone's business if I'm married or not so have never used Mrs. With colleagues or strangers.
