Military base access form in our wedding invitations

We are having our wedding on a military base and all of our guest need base access. We were given a form to give to have all our guest fill out. They need to put their name, dob, ss#, dl#, etc for a background check. We are going to include them in our invitations. How likely do you think our guest...

We are having our wedding on a military base and all of our guest need base access. We were given a form to give to have all our guest fill out. They need to put their name, dob, ss#, dl#, etc for a background check. We are going to include them in our invitations. How likely do you think our guest will fill it out and send it with their RSVP?

    I enclosed them with my invitations along with an explanation. Our guests had no problem sending them back to us with their RSVPs. I had our vendors contact the base directly. I turned them all into the base's pass and ID office and they called me a couple days later to tell me everyone was good to go.

    You mind me asking, how did you do your invitations? Do you have a sample? it seems like so much information to include in an invitation so I'm just wondering how you might've done it.

    I will be having my wedding on a military base in September and having a hard time with what the wording should be on the response card. Any examples or samples? I would appreciate any help! Smiley smile
