Keyhole Wedding Dress and Alterations

I am new to wedding dress shopping so this may be a silly question, but I just recently purchased an all lace wedding gown with an open back keyhole. I love it, but after finding the dress online I am a little worried. When I tried the dress on it was big on me, so they clamped the dress so I could see what it would look like once it was fitted. The open back looked really nice, but now that I see it online the keyhole looks much smaller on the model than it did when I tried it on so I am worried once it is fitted into my size will the keyhole remain the same size as when I tried it on or will it be smaller since I am ordering a smaller size dress? Does anyone have any experience with this?

Also is it possible to alter the opening in the back to be a little bigger? I really love the dresses that go really low and are open in the back, but I am worried the keyhole will be smaller once it is fitted. (The materials are Chantilly lace, tulle and satin straight)
