How to fill out RSVP card

I am in need of some help of how to fill these out. I have never had to fill them out myself so when I ordered the rsvp cards, I just thought I didn't know how to fill them out, but now that I've read up on them, I'm still confused. Do I need to write the guests name in the blank field or do I leave it blank for them to fill out? Also for # of people attending, would it be rude of me to write in a /5 for example if I invited a family of 5 So they could just write 5/5 or 4/5 or whatever the case may be. Both sides of the family are hispanic and I just want to make it clear that only immediate family is invited for the invitations that I will address as "Mr. Last Name and Family". From my experience, when addressed as family, we tend to invite the whole block lol. Anyway, any help would be appreciated! Disregard the date on the card, thats just an example of what mine looks like.

    Put Mr and Mrs John Smith and family on outside envelope. On inside envelope put first names of people invited. I guess you could fill that name line in for them, but people usually do it themselves. Be sure to number them on the back in pencil so in case the name is missing you know who it is from. Not sure o about doing /5 of the guests though. That might be tacky.

    I had something similar to this. For example i Invited Mr. and Mrs. Kenny Cohen and Family (they are a family of 7)

    They filled it out as:

    Mr. and Mrs. Kenny Cohen

    X next to accept

    6 next to number of guests

    i then had to call to see which child wasnt coming...

    I would write out the names of the guests on the back of the card to avoid confusion!

    My cousin's rsvp said we have reserved (X) seats in your honor.. so people got the hint they can't invite themselves with a guest LOL
