How do you write a contract for your wedding singer?

I just spoke with a lady who sings amazing and she agreed to sing at our wedding ceremony and for our first dance. I am no lawyer, but I do know putting things in writing is EXTREMELY important. Does anybody how to write a simple contract, nothing to fancy and long but straight to the point?

    Go to google and type in contract for wedding singer and it should have a lay out to exactly how a contract should look like.

    I spoke to FH who is an attorney. He said to make sure your contract includes:

    1. Exact date of event, arrival time, and location with address

    2. Attire guidelines

    3. The song(s) he/she will perform

    4. Cancellation/refund policy

    5. Fee schedule. Include fee should he/she need to fill time
