I am making my own guest book! Another project lol

Anyhow...I want to include questions on each page and I need some help!

I was thinking of putting:

1) Guest Name________________

2) Tell us 3 things to keep our marriage happy_________________

3) Where should we go for our 10 year anniv__________________
