Activity for ED

So I have a friend who's wife is a doctor. When addressing their invitation do I put Dr. & Mr. Smith or Mr. & Mrs. Smith or what? I want to respect the Dr when addressing this but not sure if there's an etiquette to follow. Advice?

Obviously every bride wants the ceremony dress to be a surprise but what about the reception dress? Just wondering what everyone's opinion is on the party dress?? Can he see it prior to the big day or should it be a big surprise as well?

We've been engaged for over 2yrs, we are now planning a surprise wedding for next May. We've got most of it planned but I wonder if anyone else has done a surprise wedding before?? Any tips, idea, or concerns that maybe we've not thought of yet??
