Maiden name to middle name process

Any married ladies drop their middle name and replace it with their maiden name? I'm looking to go from First Middle Maiden to First Maiden NewLast and this seems to be a nonissue for Social Security. PA DMV requires some extra documentation with my new name (not a court order just passport and bills in that name). Just trying to figure out if this is an acceptable change for renewing my passport. I called the passport helpline and the woman didn't seem to understand what I was asking and told me I needed to show proof of using my married name for 5 years or I needed a court order. Any input? Thanks!

    All of my married friends have done maiden to middle, here in Georgia and one in Tennessee. They brought the marriage license to Social Security to get a new card. Once the card was received they went to the DMV with the new SS card and marriage license (maybe have to bring something else if changing your address also, not sure). They all changed their passports after license because they were told you have to have a SS card and license or state ID in the name you want your passport in

    New Jersey lets you take a new name when applying for a marriage certificate.     Hi There! every state is different with name changing and every marriage license/certificate is different In Nevada the SS office will change your name to whatever you want, the DMV is the issue. The DMV does not allow maiden name to middle, this requires a court order. They will only change the last name or you can hyphenate your maiden name and new last name. Good luck!

    Hi Amber! I did this in DC with no issues! I think as long as you're changing everything at the same time it seems pretty easy. You'll already have your marriage documentation as the reason for the name change, so you should be able to change your middle and last name simultaneously!

    Did you find success? I also live in PA and am having the same issues. I changed with SS and DMV, but I asked the same question to the Passport agency and they said since I changed my middle name I needed proof of 5 years as well.

    Even thought the passport agent on the phone was being really difficult, I sent everything in to renew and noted a name change on the form. I included my marriage certificate and in about 6 weeks they sent it all back without any issues. Good luck!
