About
Informing everyone that you have a new last name may seem like a complicated process, but it really doesn’t have to be. The forms are available online—and they’re free! It’s just a matter of tracking them down and completing them. NameThatBride.com takes care of that first part for you—we’ve gathered the links to all the forms and procedures in one place. All you have to do is print, fill out, and send the forms in. There’s no need to pay an online name-change service to do any of these things for you.

Also, with this site, you can print out a checklist (to help you keep track of things) and generic "notice of name change" forms (to mail to places that don’t have an "official" form, such as credit card companies). We provide everything you need to create your own name change kit. And it’s all free! We support this site with the help of advertisers.

This website is for brides (and grooms) who have recently changed their names through marriage only. You must have a certfied copy of your marriage certificate that states your new name on it in order to continue. Get started now!

Additional Questions and Answers

Q: Are these forms limited to brides only, or can grooms also use them?
A: If the groom’s new name is indicated on the marriage certificate, he will be able to use these forms.

Q: Do I need to go through the name-changing process if I am hyphenating my last name with my husband’s?
A: Yes. A hyphenated last name is treated like a brand-new last name.

Q: Can I start the name-changing process before my wedding?
A: You can fill out the forms in advance, but you will need a certified copy of a marriage certificate that states your new name in order to complete the name-changing process. We also advise that you wait until after returning from your honeymoon to send out these forms, to avoid having discrepancies between your official documents. You may travel with a passport and plane ticket that bears your maiden name.

Q: Can I complete the name-changing process entirely through the mail?
A: Almost all agencies allow you to change your name through the mail. An exception is the Department of Motor Vehicles, which requires you to show up in person. Also, if you live in certain areas of the United States, you may be required to change your name with the Social Security Administration in person.

Q: Does it cost money to change my name?
A: Most agencies and organizations will do it for free, but some do charge a fee. You will also need to obtain a certified copy of your marriage certificate, which costs a nominal fee.

Q: How many certified copies of my marriage certificate will I need?
A: One will suffice, as agencies that require it will return it to you. However, if you’d rather not wait for one agency to return your certified copy before sending it out to another, you should purchase more than one copy.

Q: What record should I change first?
A: We recommend that you start with your Social Security card.

Q: If I start the name-changing process, do I have to complete and mail out all the forms at the same time?
A: You can go through the process at your own pace. But remember that the sooner you get it all done, the less confusion there will be down the road.

Q: Can I use these forms to change my name in a non-marriage situation? What about after a divorce?
A: These forms are meant for recently married people, but you may also use them to change your name after a divorce if your new name is stated on your divorce decree. They do not apply to other name-change situations.
